Professional Home Care's "Rapid Response Team" Orthopedic Rehabilitation Program was developed to provide the patient with a seamless transition from the hospital to the home. The "Rapid Response Team" program improves patient outcomes by using the most advanced technological treatments according to physician's protocols and national standards.
The "Rapid Response Team" is able to offer Physical Therapy, Occupational Therapy, and Speech Therapy, based upon physician's orders. Our therapists are educated in YOUR physician's protocols. Patients will receive continuing care from the same physical therapist after the initial assessment, who will design a home exercise program specifically for you.
Orthopedic rehabilitation programs include: range of motion, gait training, strengthening exercises, activities of daily living, fall risk prevention, balance, and equipment evaluation and training.
